MR Guided Focused Ultrasound Surgery for Prostate Cancer R01-CA109246

The Prostate and Focused Ultrasound Cores are developing an integrated multimodal image display system to allow accurate targeting of focal prostate lesions for MR-guided focused ultrasound surgery (MRgFUS). For this system, rapid and spatially accurate thermal imaging will be required, not only at the focal point, but at the critical normal, adjacent structures, such as the urethra and neurovascular bundles. In 2008, investigators began the preclinical tests in dogs of a transrectal MRgFUS system for the thermal ablation of prostate cancer. The system being evaluated is the ExAblate 2000 Prostate made by InSightec, Inc. Animal tests demonstrate how MRI-guidance can offer controlled and precise ablation of prostatic volumes. MRI-guidance allows controlled thermal dosimetry deposition to provide precise targeting and adequate exposure levels. Current challenges include tracking prostate motion and reducing motion-induced artifacts in the MRI.

In 2009, researchers completed canine prostate MRgFUS experiments using a prototype prostate MRgFUS device from Insightec.
